Getting Started with Proton Therapy

Your first step to determining if you are a candidate for treatment at Indiana University Health Proton Therapy Center is to have either a direct referral from your physician or make the call yourself. During your first call, we obtain your contact information and the diagnosis for which you are seeking treatment. One of our nurses or clinical specialists will return your call to learn more about your case, to obtain specific medical information, and to answer any questions you may have. If appropriate, you may be asked to visit Indiana University Health Proton Therapy Center for an onsite consultation with one of our physicians.

Registration for a Consultation

When you are ready to register for a consultation appointment, the following information is needed to start the process:

  • Demographic information on the patient.
  • If you are being referred by a physician, we will need the physician’s contact information (name, facility, address, phone, and fax).
  • Employer information (if applicable) and emergency and relative contact information.
  • Insurance information from all carriers (provider, name of the insured, ID number, group number or plan code, and Member Services telephone number). This information should be available on the cardholder’s insurance card.

You will be scheduled for a consultation with one of our radiation oncologists. Consultation appointments are generally available Monday through Friday. A registration packet will be sent to you, or you can obtain registration forms from our Website.
Our insurance staff will contact your insurance provider to determine if the physician office consultation will be covered. We will notify you of any costs you may incur for the consultation.

Preparing for Your Consultation

Your consultation will involve a physical examination and an in-depth medical records and film (CT, MRI) review to determine if proton therapy is medically appropriate for your treatment course. To prepare for your upcoming visit, you will need following items prior to your consultation:

Medical Records from Your Physician(s)/Oncologist(s)

Copies of films, CDs and reports from the past three years of radiological exams (i.e. MRI, CT scan, bone scan, X-ray, PET scan). If surgery was performed, please provide films done pre- and post-surgery.

  • All previous radiation therapy records (if applicable), including the treatment summary, isodose plan, and dose volume histogram
  • Pathology reports from each surgery related to the disease
  • Operative reports from each surgery related to the disease
  • Lab reports from the past six months (PSA, blood counts, kidney function, liver function, endocrine tests – thyroid, testosterone)
  • All chemotherapy medications and dates administered (if applicable)
  • Any medical correspondence pertaining to the disease


If possible, please have all records sent at least one week prior to your appointment, to Indiana University Health Proton Therapy Center, 2425 N. Milo B. Sampson Lane, Bloomington, IN  47408. ATTN: CONSULTATION. If timing does not allow, you may bring the information to your appointment. These records are essential for a complete consultation. Without this information, your consultation may be rescheduled for another date.

Forms should be completed in their entirety and brought with you to your appointment. For your convenience, you also can download registration forms and print them. Should you have questions while completing the forms, please make a note on the form. Our staff will review all forms with you at your appointment.

Make Hotel and Travel Arrangements

If you require overnight accommodations, several area hotels offer discounts to our guests. When contacting hotels on the list, be sure to mention that you will be visiting Indiana University Health Proton Therapy Center to take advantage of applicable discounts. Questions regarding places to stay, dining, or area activities should be directed to our patient concierge.

In addition to your medical records and Indiana University Health Proton Therapy Center forms, please bring the following items for your consultation appointment:

  • Insurance card(s)
  • Copies of all Indiana University Health Proton Therapy Center communication
  • Notebook with questions to ask your doctor
  • Payment for consultation visit, if not covered by your insurance (Visa, MasterCard, cash, or check accepted)

Your Consultation with an Indiana University Health Proton Therapy Center Radiation Oncologist

At the time of your initial consult, you will be seen by the nursing staff and one of our radiation oncologists. Once your condition has been fully evaluated, a decision will be made as to whether treatment at Indiana University Health Proton Therapy Center is an option for you. Consultation appointments can take anywhere from 30 to 90 minutes and depend upon the individual’s medical history. If proton therapy here is your selected treatment option, insurance approval is obtained and treatment planning begins. 

Some cases are not appropriate for proton therapy. In those situations, we may be able to recommend other treatment options.
